At Acquaintance Care, we provide Assist-Life Stage, Transition supports as part of our Support Coordination services. Our focus is on helping participants navigate the NDIS, connect with the right providers, and manage supports in a way that is structured, consistent, and goal focused.
We work closely with Support Coordinators, families, and allied services to ensure participants receive coordinated, practical, and responsive support across all areas of their life.
This service is suitable for participants who require assistance with managing multiple supports, life changes, or increased complexity in their NDIS plan.
What we do as part of Support Coordination
Support Connection
We assist participants to build and maintain effective connections with NDIS providers, mainstream services, and community supports. Our goal is to ensure supports are stable, appropriate, and well matched to the participant’s needs.
Coordination of Supports
We manage and coordinate multiple services so they work together effectively. This includes communication between providers, monitoring service delivery, and supporting consistency across all supports.
Capacity Building and Independence
We focus on developing the participant’s ability to understand and engage with their supports, increasing independence over time and reducing barriers to accessing services.
Life Stage and Transition Support
We support participants through key transitions such as moving out of home, changing providers, starting new services, or adjusting to changes in their routine or environment.
Accommodation and Tenancy Support
We assist with understanding tenancy obligations, maintaining stable housing, and linking participants with appropriate accommodation supports where required.
Decision Making Support
We help participants understand their options and make informed decisions about their supports, services, and future planning.
Goal-Based Planning
We work with participants to develop clear, practical goals aligned with their NDIS plan and ensure supports are structured around achieving those outcomes.
Crisis and Service Breakdown Support
We respond to urgent issues, service disruptions, or breakdowns in supports, working quickly to restore stability and continuity of care.
